There are two valid parts to your answer: Magic and It's a Fantasy Location. With Magic cleaning and a lack of industrial parts can easily be overcome. Because It's a Fantasy Location there are a lot of parts to the culture of the area that are simply unknowable to us especially so with Magic involved. As such we can't expect the ladies undergarments to appear as they would in our world's pre-industrial civilizations. Nor can we expect them to be as filthy and disease ridden.
The question that appears before my eyes: How do they make these ridiculous underwear types if they don't have a modern industry to create them in the first place?
Female underwear in non-industrial settings should look more similar to diapers.
Also, let's not forget that they probably don't know soap, so their underwear and bodies will probably smell a lot (and be pretty dirty)...
I for myself, with my knowledge about pre-industrial civilizations, wouldn't even want to catch that stinky and most likely still damp "panties" with my hand, god knows what disease I might catch if I do as a bonus...
haha actually in many stories i like more the specials anthology when only comedy appear. this one for sure follow.
