I just recently discovered this series and so far, I like it. A breath of fresh air from the typical protagonists in my opinion. The lingering question is "what is wrong with him?" I don't think there is anything wrong with him. It's just that his personality is not the "typical MC".

 

He is not a sociopath either (obviously the description of this psychotic disorder does not fit him), I can say that he is INDIFFERENT and you don't often get an MC like that on the get go that is why people perceive that there is something weird about him.

 

His personality shift after the torture was also reasonable, it's out of the norm, but it is a possible reaction after a traumatic event. In the beginning of the series, we can see him as a proud, uncaring person, but upon being rescued, he suddenly had this sad, fragile look indicating a loss of power. The series of torture humbled him and made him realize that things are beyond his control. He was experiencing depression and that justified his refusal / lack of desire to kill his tormentors. A depressed person doesn't conform to anything. But his personality resurfaced after realizing that he is about to be killed by the hat guy. It was Kei regaining his old self. Back to the non chalant guy who decided to save the two medical staff out of his own selfishness because they will prove to be useful for his escape. That is why the next chapter's title was "The Real Nagai Kei". He has shed his armor of indifference, he has recovered from a tragedy, and finally managed to interact with the black ghost because he has fulfilled the requirements for a power up. (yeah, like a friggin game) 

 

So yeah, the bottom line is, nothing is wrong with him, the author just created a main character with a definitive personality.

I think part of the thing with Kei is the author shift; the original author probably had one idea of what Kei's character was, while the artist had another. Though I also agree that the torture also would have affected him, plus he does seem to have a calculating and adaptive element to his character.
