In japan, sticking out your thumb or pinky while speaking to someone in reference to someone else is a way of asking "Are they your girlfriend/boyfriend?" If I recall correctly, it's thumb for boyfriend and pinky for girlfriend. So that's what she's asking him by sticking out her pinky. his response (With the thumb down) is, of course, no.
