Unless Tooru is a masochist who likes to stab herself with car-sized knives/swords, the scale is fucked up.

 

And I did notice the tail/bust size changes but most of them were simply cases of perspective and situational comedy.

Just to be devil's advocate...

Considering Tooru was cursing the gods when they first met, the chances that they were related to the sword in some way is pretty high and we know absolutely nothing about them. You're assuming for some reason a human stabbed Tooru and therefore the sword should be a scale for a normal human to wield but for all we know the gods could be forty feet tall and that's just a tooth pick they accidentally dropped.

That said, anyone just needs to look at this artist's other works to see that they have a problem with consistency. Trying to find plot reasons to justify those discrepancies is pretty pointless.

Considering that her proper dragon form is larger than the top of Kobayashi's apartment building, but can also make herself small enough to easily fit within a single apartment room in dragon form suggests that she can control her form, size and shape at will. You didn't notice that her bust and tail sizes keep changing as well. The only thing that I don't think has changed in human form has been the size of her horns, something she probably has little to no control over.
