She originally thought there was water inside that little hull, and i'm pretty sure she was right about that. What Kasuga did was pour oil into that little hull, however oil floats on top of water. So despite the fact that he has lit up the oil, it is only the surface of the liquid inside the hull that is lit on fire; while below that surface of oil is safe water.

Regarding chapter 23, the way Miyamura avoided death by hanging is preposterous. Ripping open your trachea isn’t really gonna alleviate the problems caused by the fact that the rope would still pinch off your common carotid arteries which supply your brain with blood (and thus oxygen). In other words, she would still suffocate. So unless you’d go for some idiotic excuse à la “Derp derp derp, the virus makes them invincible” this here is another glaring plot hole.
