Here's a kind of explanation: Maribel can freely move through boundaries in her sleep, leading her to Gensokyo, Makai, the Lunar City, and so on.  When she's in her dream-like state, her actual body is in those places.  She's repeatedly "brought back" stuff from when she's gone to places in her sleep.  Well, in this case she's been raped by a youkai or something and came back to the normal world pregnant.  Now, her power is strikingly similar to Yakumo Yukari's and not to mention they greatly resemble each other, so the fetus is very likely that of her.  This is actually a very dark doujinshi but treated with the very lighthearted flair of the Touhou series.  A very befitting doujinshi if you ask me.
