Dream or not, the author devoted several chapters to this shit twist. Not just that, but I didn't read Genshiken, I don't know any underlining reasons or whatever, I know these characters and what they have shown us so far. What I see is an asshole insecure husband cheating on his wife. That's it for me.

Madarame is/was??? one of my most all time favourite anime boys. I know this is !Madarame but this was very disturbing to read. I'm actually PISSED.
Mind you I am a fan of Kio Shimoku and have read their other works like Yonensei and Gonensei. He has trolled in parts of his other manga. Stuff like a

Whatever the reasons, it doesn't change the fact that Madarame is truly a pathetic person and I have zero pity for him.
In other news, Ch 23.5 looks like alt-Ogiue finally appears. !Sue dotes. The editor, Endou is trying to figure out !Hato's backstory because she wants to bring !Hato's work mainstream. !Yajima continues to be plagued by insomnia.
As for THAT chapter: Some wag philosopher said the greatest compliment a writer can give their readers is to frustrate their expectations. Effing harsh. I blogged about it, because it succeeds at pissing off the old guard guy fans AND the few western fujoshi that liked the series. No idea about Jp. fujoshi.
A few points I noted:
Jealousy is hardly it: !Mada is scared shitless of fatherhood +responsibility. Pus !Saki is now ruler of the family - she is even assertive about the cosplay thing; which ruins it - she's supposed to be flustered by it to turn him on. Lotsa Jp stereotypes on absent drone daddy being almost outsider in own house- even the pets forget who he is.
!Mada eyes wide open. Reads "trying to seduce me" into a text message. !Hato can't refuse. Bakka!
Don't look at what Kio did, look at how he did it. That ain't yaoi, it aint otokonoko/josei (Dicca), it is ...thorough, mechanical, dis-enchanted. !Mada is brought short when doggy style reminds him of his wife but then has no problems as !Hato rides on top, re-enacting !Saki's honeymoon taking of him. (per vol extras)
Note the abbreviated 1970's bishonen vow of love from !Hato, who is in fugue mode. While the !Mada gets uked part is offstage, there is no indication that !Hato went all S2 Genshiken anime ep5
The proceedings were "staged" by Kio so that "Everyone will be equally dissappointed"
Finally: Kio has a new audience. Spotted was idling in Le Paradis' driveway. Now it gets page count, so it has to be a proper Josei manga. Marital Infidelity! Flawed Husband. !Yajimacci in a Maka-Maka almost lesbian friends with benefits but !Yaj gets the short end of it relationship. Early 30' couples going through relationship troubles with fading fan enthusiasms of their youth.
These were supposed to keep the riajuu world "re-enchanted"

Serves me right for expecting anything good from the author of Genshiken.
It doesn't matter that Sue doesn't have an ex. I'm talking about Madarame, who has so little self-confidence and self-control that he'd cheat on a wife who obviously loves him. In this case he was jealous of Kousaka, and in Sue's case, if she becomes the uber hot adult depicted in this manga, he'll be jealous of the random dudes that would be attracted to her. It's not a problem with the girls, it's a problem with MADARAME.

The difference in the Sue end is that she doesn't have an ex for Madarame to feel jealous about. Plus she's his favorite type (apparently).
It was toeing the line, but if he didn't actually do anything then I wouldn't consider that betrayal. It was obviously his inferiority complex that brought this on when he saw her with her ex. Could this bring drama? Sure, but if it was a dream, I don't see her leaving him or being mad long. I'm sure will have more inner turmoil than anything.
Considering that the husband wakes up on the couch with his clothes on, and not naked on the bed, it is possible that it was a delusional dream. Also, his last lines were something to the effect of, "...but does this constitute betrayal?" which I interpret as him wondering if even dreaming about betraying his wife is actually doing so. Of course, the mere fact that he invited her over with lust in his heart is a de facto betrayal.
Notwithstanding, I don't care if it was a dream or not. In my opinion, the whole scenario was out of place for this story. We haven't even seen such intimacy between the husband and his wife, and certainly not in such detail.
There's a possibility its a dream? See if it turns out to be a dream, then I'd take that over him actually cheating on her. It came out of nowhere and despite having attraction problems when she was pregnant, he really seems to love his wife. People have fantasies. Him dreaming about doing someone else could make sense considering he's close with that person and aware of her feelings for him plus after he had those negative thoughts about seeing his wife with her ex, it's most likely that his inferiority complex could have caused it.
Kinda hope it was a dream/fantasy because I really like this manga.
After "reading" chapters 22-23, I've gotta say that this is one of the biggest disappointments in a story that I had really enjoyed reading, regardless of whether it was all just a dream or not. Granted, it is a completely realistic scenario, and probably happens all the time with many husbands who have pregnant wives and wives who have just given birth, but it was certainly not something I wanted to see happen in this otherwise charming story, especially in such graphic detail. If I wanted to see read some hentai, I certainly wouldn't come to batoto to do so.
Why would the mangaka do such a thing that could alienate a large portion of the story's readership? Probably reliving his own fantasies.

The problem with this "what if" is that it feels like he's just trolling at this point. Primarily, the twist only makes sense if you know the context of the characters from Genshiken. Basically, if this was purely a non-canon arc, fine, but then don't insert so much of from the "main" story (Genshiken), just leave the characters ambiguous and tell a different story, whatever floats his boat. The fact that he clearly and blatantly called out that these characters really are the Genshiken people means he's not just flipping the table in this story but tying into Genshiken itself which cheapens both, in my opinion.
The whole hospital scene with the "ex" being in the same room has little emotional impact unless the reader is heavily vested in the characters from the main manga. He knows that readers have been speculating that these people are really Mado and Saki this entire time, so he deliberately creates an emotional payoff for those people by having an important scene with the "ex", then he shits on those feels immediately afterwards. It also shits on readers who were just following this arc as an otaku and normal wife struggles slice of life. It's a double-shotgun blast of "fuck you" to the audience regardless if it's a dream or whatever at this point.

Wew. Way to ruin a nice story. I can only see this end badly, so I won't bother.
