Disagree. I've seen ones where that doesn't happen and they are smutty.

Unless there's a smut tag next to the shoujo tag - in which case the male lead starts out being an asshole (and a rapist) and remains an asshole (and a rapist) till the very end.
There are quite a few of those where the male lead isn't an asshole, but even if you have ones where he's an asshole at the beginning, usually he changes throughout the story. It's very rare for him to stay an asshole.
This is like a fireman version of P to JK.

Si mi memoria no me falla, ya existe una con un doctor. No recuerdo su nombre porque la lei hace ya muchos años atras, pero es basicamente una Gal que despues de un accidente termina en silla de ruedas de forma permanente, y ella se enamorando de su doctor a cargo, el cual es un joven que solo se dedica a su trabajo y no tiene tiempo para chicas.
