I have not looked through all of the comments, but I was struck by the note about 'o-tanuki' in chapter 39. While the term may be appropriate, especially in that context, as a reference to the crafty nature of Takamori, there may also be a double meaning. Tanuki are known in mythology to use their large scrotum for various amazing feats of magic, which would match well with the image of scrotal hydrocele.

I must say, while I ordinarily don't go for historical manga because I find it boring, this one is damn good. I saw very little unrealistic ridiculousness, save for the time travel bit and all the convenient sicknesses that let the doc show his prowess.

 

This is competence porn at its finest, plus a nice dose of nuanced, satisfying drama that details the unforgiving life in the time period.

I don't want tropes, I am just curious about that whole situation with poisoning. As of today we are left with cliffhanger and waiting about year or more to see ending of this arc.

I actually enjoy more having the physical copy.

I can understand that, though I've largely moved to Kindle for my off-line reading.  There's too much clutter in my house as it is (though the majority is my wife's).

 

Well, we can live in hope.  This is a good series, and it meets most of the criteria publishers want for books to be published in English.

Maybe, maybe not. This is a very long series.  I don't suppose you've tried translating or scanlating for yourself?  It is time consuming, and far from easy.

 

Isn't Patreon accessible worldwide (excepting perhaps certain countries like China, with its Great Firewall)?

 

This isn't your typical corporate licensing arrangement, where the translation is only available within certain countries.

I have no doubt that this series is more difficult to translate than your average action manga, on account of the specialized vocabulary you mentioned.

Yeah, not translating the currency as a modern day one sure is difficult.

I'm not disagreeing with you on that count, but this is a series with a whole mess of medical terms to deal with, and such things are not often found in detail in the dictionaries us gaijin normally have available to us.
 
FWIW, I was approached by two different people who don't know each other about taking up this series for scanlation.  Both were concerned about the rate of scanlation of this series.  One of them is a doctor who felt he could contribute to the quality of the scanlation as well.  I said no, for now at least, at first because Easy Going Scans does a good job and I wanted to give them more time, and because I had and have a full plate already, but then later also because the mangaka now has an official release from which he is actually earning money.  Though it is not being done through the normal "publisher license" model, I still respect the mangaka and wish to support him.  See earlier postings for details about that.

why is the currency translated as "dollars" ?

I dug up volume 9 from my basement.  The results:
 
The unit was 両 (ryou).  From Wikipedia:

Following 1868, a new currency system based on the Japanese yen was progressively established along Western lines, which has remained Japan's currency system to this day.

Immediately after the Meiji Restoration in 1868, previous gold, silver and copper coins, as well as feudal notes were allowed continued circulation, leading to great confusion. In 1868, the government also issued coins and gold-convertible paper money, called Daijōkansatsu (太政官札), denominated in Ryō, an old unit from the Yedo period, and private banks called Kawase Kaisha were allowed to issue their own currency as well. Complexity, widespread counterfeiting of gold coins and feudal notes led to widespread confusion.


As for why it was translated as "dollar", I can only speculate that the translator was from the USA, Canada or Australia.

So far, six chapters have been posted there, with a new one being posted every week.  The web site claims there are some 67 readers paying to read beyond chapter 3 now (the first two chapters are free).  At US$5.00 per month, and 4-5 chapters per month, it is only a hair more expensive than buying the tankobons would be in Japan.
