From what I can read of this, it seems like the present form (with 飛ぶ instead of 吹っ飛ぶ) is largely equivalent to "he's got a few screws loose". So that seems like it's basically saying he got a few screws loosened.

Now I'm trying to figure out what the idiom 頭のネジ吹っとんだ actually means... I'm assuming the implication is subtly different from the "he's got a few screws loose" that comes across in English.
