A few of the extras seem to be missing from this list, two of which can be found on other sites. It seems like at least one hasn't been translated, since the Extra series "The Girl's Situation" goes from 1 -> 2 -> 4.

it's 多元重奏飽和砲撃 Tagen Juusou Houwa Hougeki “Multi-Ensemble Saturation Bombardment”, and I don't think it exist in any Fate series / arc / zero ... dunno where Type-Moon obtain this uber powerful mahou loli attack ... Nanoha ?
